Set up an Account
Equities
Important, print and keep these instructions.
Step 1; Read:
Read the Corporate Profile, Financial Services Guide and UBS Account opening Terms & conditions. Read as much or as little as you like, but keep these important documents.

Step 2; Fill in and sign paperwork:
The Application forms.
UBS Share trading account application form (part 1 - Acknowlegement)
UBS Share trading account application form (part 2 - Account Details)
Please fill in your details as complete as possible, and remember to Sign on Page 1 and 5. (if in doubt about any item on the application form leave it blank and I can ask you the details over the phone)
Most important are your/all contact details, date and place of birth, Tax File Numbers, Drivers License Numbers and Passport Numbers (if you have one).
Step 3: Immediately send the application form paperwork. Either:
Fax it to: 07 5574 3881
Post it to: InvestMasters, P.O. Box 4983 GOLD COAST MC Qld 9726.
Step 4: Also important - ID:
You need to post us a certified copy of the following ID to InvestMasters, P.O. Box 4983 GOLD COAST MC Qld 9726.
Certified copy of your Drivers Licence
Plus: Certified copy of your Passport
Or Certified copy of your Birth Certificate,
Or 3 of the following bill/rates notice/bank statement with your current address on it.
Note: ID needs to be certified by a Justice of the Peace, or a solicitor, or a pharmacist; this can be done at any solicitor’s office, police station, court house, chemist, and most supermarkets.
You may need more help with this, don’t hesitate to ask for it!

Margin Lending
Step 1; Read, Fill in and sign paperwork:

Leveraged Equities Margin Loan Application form - a margin loan is used as a Bank Account and Holding account for all accounts activated for Options and Warrants.
Please fill in your details as complete as possible, and remember to Sign wherever it says "Borrower" . (if in doubt about any item on the application form leave it blank and I can ask you the details over the phone)
Step 3: Immediately send the application forms paperwork. Either:
Fax it to: 07 5574 3881
Post it to: InvestMasters, P.O. Box 4983 GOLD COAST MC Qld 9726.
